Field Trip/Event Expectatiions & Informed Consent Form

Should any department plan to conduct a class related field trip or University sponsored event activity they need to have all participants review the University’s Field Trip/Event Expectations Form and complete the Informed Consent Form. A copy of these forms should be kept on file at the department. The University's Field Trip Policy can be reviewed at (Field Trip Policy Manual 6910-C).
