Prompt Incident/Accident Reporting To Risk Management
Our Risk Management & Insurance Office needs all BSU Departments to report any incident or accident, whether it is for worker's compensation, automobile, property or liability to our office within 24 hours, wherever possible and no later than 48 hours. In that way, we can investigate the accident promptly, and take the necessary steps to process any applicable insurance claim(s) to our State Insurance Administrator.
Please do not wait to report an incident or accident until have you all available information or for the receipt of repair or replacement cost estimates on property. Send your report with the available information you have upon your first notice. We can follow up on the needed additional information or property cost estimates after we receive the incident or accident report.
Worker's Compensation - Accidents caused by your work activity with BSU
All University employees are required to notify their immediate supervisor whenever they have a work injury or employment caused illness, as soon as possible. As noted above, that should be done within 24 hours, wherever possible and no later than 48 hours from the time of the accident.
University supervisors are then required to complete a Worker's Compensation Supervisor's Accident Report form (SAR - MS Word required to view) and e-mail (first preference) or mail it (second preference) to the University Risk Management Office. That report should be sent to the Assistant Risk Manager, Breck Skinner at breckskinner@boisestate.edu with a cc to jcoffey@boisestate.edu. That report is designed to be completed by University supervisors or managers, not department employees.
When University employees report their work caused injuries or illnesses promptly to their immediate supervisor it helps protect their worker's compensation claim filing rights.
All worker's compensation injuries or illnesses that involve at least one (1) day lost time from work or that require medical treatment at a clinic or hospital requires the Risk Management Office to submit an employee claim notice to the University's Worker's Compensation insurance carrier, the Idaho State Insurance Fund within ten (10) calendar days of the University's knowledge of the accident.
BSU's Worker's Compensation Preferred Medical Service Provider is Primary Health Services. Employees with worker's compensation injuries/illnesses should go the nearest Primary Health Services clinic for medical treatment. Again, this form is designed for University personnel use, and is not for University students.

While none of us want accidents to happen, they do occur and our Risk Management and Insurance Office needs to be notified promptly so we can act on behalf of the University, as needed.
